Breast surgery can be and is done under local anesthesia, not only with the patient comfortable and painfree, but wide awake. Breast biopsies are frequently being done that way. And breast implant surgery is, too. This would be safest. And the person having a minor procedure like a breast biopsy can even drive home afterwards.

Breast implant surgery of both breasts is usually done with some sedative and narcotic as well as local anesthesia, or with general anesthesia.
